activation issues...

i had virgin mobile for a number of years. i switched over to t-mobile (and t-mo sim card) for a few months, and now want to switch back – keeping the same number i've had all along.

it is my understanding that i need to activate my phone again, but when i went to the activation page, i got as far as the screen asking for my iccid. it had an incorrect number in the box, so i replaced it with the correct one, but it does not allow me to click "next."

please advise.
