You can call boost at 1-866-402-7366 and inform them you lost your phone so they can  cancel it for you.

They can also send you a sim card for free you just have to wait 3-5 days on the shipping and is business days 🙂

If you have insurance you can make a claim at this number 1-844-534-3099 and depending on what phone it was you would pay for a deductible they range from 20,50,100, or 175.
