-Phone number that you want to transfer.

-Pin code.

-Account number.

-DEC or IMEI of the Boost phone. 

-Your name and last name.

-Plan that you want to have.

-A 4 digit pin for your Boost account. 

-Your full address.

Port problem

I have a new boost phone and went online to activate it, but it’s saying my number (Verizon) isn’t eligible for porting... haven’t had this happen before.
